Required if you plan to purchase controlled substances:
- DEA License
- DEA Due Diligence Documentation (Ink signature required. E-signatures not accepted.)
Required if you plan to purchase tax-exempt products:
- Wholesale Account Sales - Sales Tax Exemption Form (based on your state)
- Nexus Form (If applicable)
